Online referral form allows NDIS participants, and their formal and informal carers to submit a request to access Dot Humanity Support Services. We are an NDIS registered provider and provide supports to NDIS participants in Western Australia.
Online referral form is an alternative request to access our support services. This online request form provides flexibility to NDIS community to have an alternative request option if they can not use or access their MyPlace portal.
We will ask you to provide some basic information such as your name and contact details and what services you requesting. We may also ask you some information about your NDIS plan.
